Will a Better Understanding of the Problem With Transformed Follicular Lymphoma Lead to Better Outcomes?

Along with early treatment failure after immunochemotherapy, transformation of follicular lymphoma (FL) to high-grade lymphoma—prototypically diffuse large B-cell lymphoma—represents the greatest fear of patients and their medical teams. In medicine, potential threats such as this one are best addressed by initially developing a thorough and up-to-date understanding of the problem. During the past 3 years, the clinical details of transformed FL have come into somewhat sharper focus with the help of three reports on large prospective observational studies. Of particular interest are details regarding the frequency and timing of transformation events, clinical findings at presentation that are predictive of transformation, management strategies at initial diagnosis that favorably or adversely impact the incidence or outcome of subsequent transformation events, and prognosis and the best management options after transformation.
