Eliminate Prophylactic Platelet Transfusions?

Studies have clearly shown that the threshold for prophylactic platelet transfusions can be reduced from the previous guideline of 20,000 platelets per cubic millimeter to 10,000 per cubic millimeter and that the standard platelet-transfusion dose can be reduced by half, without increasing the risk of bleeding in patients with hypoproliferative thrombocytopenia. The next logical question, addressed in the study by Stanworth et al. in this issue of the Journal, 3 is whether prophylactic transfusions are really necessary or whether patients with thrombocytopenia can be effectively supported with only therapeutic platelet transfusions given with the onset of a bleeding event of World . . .
