Chronic lymphocytic leukaemia is characterised by a considerable heterogeneity in clinical course. Although the clinical staging systems described by Rai and Binet about 40 years ago proved to be robust prognostic tools, improvement was needed, especially in view of the increasing proportion of asymptomatic, early stage patients (Rai 0 or Binet A). Many novel biological, genetic, and molecular prognostic markers for early stage chronic lymphocytic leukaemia have been described during the past decades, but for most of these, prospective validation within clinical trials is missing.
