Great progress has recently been made in the treatment of chronic lymphocytic leukemia (CLL) with US Food and Drug Administration approval of several highly effective, orally administered small-molecule inhibitors of B-cell receptor signaling and apoptosis pathways. Whereas these agents, arguably, are breakthroughs in therapy, most patients achieve partial remission as the best response, and, if tolerated, these agents must be administered continuously until progression. Not an insignificant proportion of patients ultimately discontinue therapy as a result of toxicities and intolerance.
