Metastatic Prostate Cancer

Care for men with prostate cancer is a major global health care challenge, compounded by an aging population and increasing frequency of diagnosis. The priorities today are similar to those in the recent past: minimizing overtreatment of indolent disease and improving outcomes for patients with aggressive disease. Herein we focus on recent accomplishments and future challenges in the management of metastatic disease, which continues to be associated with a high rate of death despite multiple new-drug approvals in recent years. Metastatic prostate cancer can be broadly divided into two groups: disease that has not been treated with androgen deprivation and disease that is resistant to such therapy. Treating metastatic prostate cancer is becomingly increasingly complex. We review studies that are changing the standard of care, and we offer a conceptual perspective for addressing ongoing challenges and opportunities.
