Precision medicine in the adjuvant treatment of gastric cancer
Menée en Corée du Sud à partir d'échantillons tumoraux fixés au formaldéhyde et inclus en paraffine après prélèvement sur 307 patients atteints d'un cancer de l'estomac de stade II à III traité par gastrectomie D2 en combinaison ou non avec une chimiothérapie adjuvante à base de fluorouracile, puis validée à partir d'échantillons tumoraux prélevés sur 625 patients complémentaires, cette étude évalue la performance d'un test, basé sur l'expression de quatre gènes associés aux caractéristiques des tumeurs gastriques (gènes GZMB, WARS, SFRP4 et CDX1), pour prédire la réponse à une chimiothérapie adjuvante
Adjuvant chemotherapy is considered the standard of care after surgical resection in patients with stage IB–III gastric cancer in most Asian countries. The benefit of adjuvant chemotherapy after surgery over surgery alone in terms of overall and disease-free survival was confirmed in an individual-patient data meta-analysis1 of 3838 patients treated in 17 randomised controlled trials. After surgery with curative intent, patients with gastric cancer receive postoperative chemotherapy to eradicate undetectable micrometastatic disease.
