Creating a bridge for transition: From pediatric cancer survival to life-long, risk-based health care of the adult cancer survivor

Long-term, risk‐based health care focused on education and screening can prevent or mitigate late complications of childhood cancer treatment, but less than one‐third of patients access this care. To enhance awareness of risk and the need for ongoing care, it will be necessary to close the 5‐year gap from diagnosis to risk‐based health care (survivorship), provide timely education and cancer treatment summaries, and offer robust models of care that allow a transition to an adult medical care system that maintains access to expertise in the consequences of surviving pediatric cancer.
