SMART: logical radiotherapy and surgery beyond MARS

The most highly anticipated surgery trial for malignant pleural mesothelioma since the MARS trial is the mature follow-up of the patients receiving oligofractionated hemithoracic radiotherapy followed by extrapleural pneumonectomy by B C John Cho and colleagues published in The Lancet Oncology. MARS, although provocative, forced mesothelioma surgeons to question the role of extrapleural pneumonectomy in trimodality management of pleural mesothelioma, and concluded that extrapleural pneumonectomy within trimodal therapy offers no benefit and possibly harms patients. Therefore, any subsequent trial should be examined carefully with regard to the MARS conclusion.
