Who Should Undergo Transplant for Unresectable Colorectal Liver Metastases—Finding the Needle in the Haystack

Over the last 2 decades, the combination of surgery and modern chemotherapy have resulted in 5-year survival rates of approximately 50% for well-selected patients with liver-confined metastatic colorectal cancer. Although the majority of these patients will experience a disease recurrence, approximately 20% appear to be cured at 10 years. These somewhat unexpected results have generated significant interest in a revival of the concept of liver transplant for patients with unresectable colorectal liver metastases. Initial experience with liver transplant for this disease was met with dismal results, and the concept was largely abandoned. However, a group of researchers from Oslo, Norway, has truly revitalized this concept.
